Walk-In Care

Osceola Medical Center and Walk-In Clinic provides walk-in hours for convenient and quick care. The clinic sees patients with illnesses or injuries that need to be treated right away. Use the Save My Place service to save your place in line for walk-in care.

If you have a minor injury or illness, just walk into the clinic with no appointment necessary. Our qualified medical staff treats patients ages 3 months and older.

Sports Physicals

Middle school and high school athletes must complete a pre-participation physical evaluation before starting a camp or practice. The Missouri State High School Athletics Association (MSHSAA) requires the forms prior to the first practice. CMH's board-certified physicians, nurse practitioners and physician assistants administer the physicals.

CMH Clinics recommend that your student-athlete complete an annual wellness examination and have the sports physical included in the appointment. During summer sports physical days, your athlete can receive a free sports physical at Osceola Medical Center.

Share or Request Medical Records

Request your medical records online.

Records also can be requested by completing and returning the Release of Information Authorization Form PDF via mail or fax. A medical record copying fee will apply. Please allow at least 3-5 business days to fill your request.

Securely upload and share medical images with CMH providers using the PowerShare website.

Upload medical records to your provider using our secure upload utility. You can also fax medical records to 417-328-1110 or call 417-328-6304.

  4. If your insurance requires a co-pay, please bring this with you to your appointment.

Please arrive at least 15 minutes prior to your scheduled time to allow for check-in and paperwork. If you arrive after your scheduled time, you may be asked to reschedule.

Yes, if you have a Patient Portal account. You will receive an email one to two days before your appointment with a link to complete your registration online. You also can access the paperwork by logging into the CMH Patient Portal.

Easy and secure access to all of your medical information, results and appointments is available through the CMH Patient Portal. This is also an easy and fast way to communicate directly with your provider or care team.

You can message your provider or nurse directly through your CMH Patient Portal. Messages are viewed during normal business hours.

If you have a question during business hours, you may call and leave a message for a nurse. Calls received after 2:30 p.m. may not be addressed until the next business day.

If you are not able to keep your appointment, please give as much notice as possible; 24-hour notice is preferred. After missing three appointments without canceling, you may be limited to same-day appointment availability for up to one year.

All CMH primary care locations offer same-day OR walk-in hours for acute, urgent illness and injury. Walk-in hours vary by location. Same-day appointment openings are usually reserved for urgent illnesses or injuries. We recommend calling early in the day as these can fill up fast.

Refills to already prescribed medications can be requested 24/7 through the Patient Portal. You also can call your pharmacy to request the medication. Medication refills require 2-3 business days to refill. Some medications may not be refilled without an appointment.

Yes. These services include chronic care management, care coordination services and community resource specialists.

Chronic Care Management: Additional support for patients with high-risk or more complex health needs. Ask your primary provider about care management services.

Care Coordination Services: A care coordinator assists with specialist referrals and diagnostic services needed outside the clinic. The care coordinator also assists with insurance authorizations and can be reached by calling your clinic.

Community Resource Specialist: A team of community resource specialists is available to help connect you with:

  • Transportation to medical appointments
  • Cost of health care
  • Dental care
  • Affordability of medications, food, utilities and housing needs resources.
