Services Provided
CMH Health & Wellness Clinic is a vital resource for people without a primary care home.
Services include:
- Medicare Annual Wellness Visits for patients without a primary care provider.
- Hospital transitional care services. The clinic gives ongoing medical support to patients after a hospital stay or visiting the emergency department.
This clinic does not provide walk-in services. For walk-in care, patients can go to the CMH Walk-In Clinic at 2230 S. Springfield Ave., Suite H-J, Bolivar.

The clinic serves all patients regardless of their ability to pay. Discounts for essential services are offered based on family size and income. CMH has a financial assistance policy and a sliding fee discount program.

Medicare Assistance
