CMH Foundation long-term care facilities are committed to providing quality care to all residents. This facility received the Bronze — Commitment to Quality Award. The American Health Care Association and National Center for Assisted Living give the award for meeting quality and service standards.
Services Provided
The facility includes a 20-bed memory care unit to meet the needs of individuals with dementia, including Alzheimer’s disease. AGHCF is in a technologically advanced healthcare complex, including Ash Grove Family Medical Center, independent living apartments and a pharmacy.
Rehabilitation Services
AGHCF also offers inpatient and outpatient rehabilitation services. Speech, physical and occupational rehabilitation are provided by full-time, in-house therapists.
Memory Care Unit
The memory care unit is a caring place for those with Alzheimer's or other memory issues. Their needs are unique, and our team is here to give personalized support in a warm environment.
Our staff is trained in dementia care, giving residents kind and understanding help with daily tasks and activities. The unit is a safe and cozy home with features that ensure everyone feels secure. Our team is available 24/7, providing continuous support for both residents and their families.
Nutritional Services
The nutritional services staff work with a registered dietitian to provide meals that are sensitive to taste, preferences and diets.
Activities
Planning and research go into the daily planned activities for residents at AGHCF. Our staff provides exercise classes, outings, crafts, religious opportunities and entertainment from musical groups for all skill levels.

Independent Living
The Ash Grove Independent Living apartments allow seniors to remain independent in a home of their own. Health care professionals are nearby. Apartments are two-bedroom, two-bathroom units with a full kitchen, electrical appliances, living room, covered patio and oversized one-car garage. The apartments are located on the same campus as AGHCF, Ash Grove Family Medical Center and the local pharmacy.

Responsible for your care and supervising the nursing staff.

Coordinates the interdisciplinary team (including physicians), develops your care plan, and schedules meetings with you and your family.

Assists with admissions, advance health care directives, and discharge planning. She receives complaints and compliments and is a liaison to the Resident Council.

Responsible for supervising housekeeping and laundry services, ensuring the cleanliness of the facility.
Matt Branstetter
Director of Plant Facilities
Maintains the condition of the facility and makes repairs as needed. Lyle is also responsible for monitoring the safety of the residents and staff as the OSHA VPP Safety Officer.
