The Citizens Memorial Health Care Foundation is a 501(c)(3) not-for-profit organization dedicated to Citizens Memorial Hospital(CMH) and the enrichment of the communities we serve.
Our vision is to be the first choice for customer focused healthcare to every generation. Your generous donations directly impact the lives of each family served by CMH and support essential initiatives such as the Carrie J. Babb Cancer Center, CMH Hospice, student scholarships and more.
There are several ways you can help keep your community healthy. You may choose to contribute cash, stocks, bonds or real estate to Citizens Memorial Health Care Foundation. Gifts to the Foundation typically provide donors with a tax deduction and nonprofit investment incentives.
Recognize someone you love or a person who has made a difference in your life. Many people choose to remember a loved one by making a financial gift to the Foundation in lieu of flowers. The Foundation acknowledges each gift with a letter to the family and the contributor.

CMH Heroes for Hospice 5K/10K Run
Be a hero! The Citizens Memorial Health Care Foundation’s “Heroes for Hospice” 5K/10K Run is the first Saturday in May. Both courses begin in front of the CMH Community Rooms and conclude with a loop around Dunnegan Park. The race begins at 8:30 a.m. and is electronically timed for accuracy. Medals are awarded to overall male and female in both races and to first place male and female winners in each age division. Participants dressed as their favorite superhero is encouraged! Walkers, strollers and pets are welcome!
CMH Hospice caregivers strive to help patients remain as comfortable as possible at home, or in a homelike setting, with family and friends. The program also provides support to the family after a death of their loved one through a bereavement program. Proceeds from the race will provide donations in our area to ensure that physical, emotional, spiritual and bereavement care is given to all terminally ill patients and their families.

Medical Excellence Golf Classic
The Citizens Memorial Health Care Foundation Medical Excellence Golf Classic is held the first Saturday in June. The classic is an 18-hole, four-person scramble at Silo Ridge Golf and Country Club in Bolivar. All monies raised goes to the Medical Excellence Scholarship.
The scholarship provides financial assistance to individuals from the CMH service area who are working toward careers in healthcare. Upon completion of their education, Medical Excellence recipients return to work at CMH.
The tournament will feature two shotgun starts at 7:30 a.m. and 1 p.m. Each player will receive a gift package, breakfast, lunch, snacks and a barbeque dinner after the tournament. Green fees and cart rentals are included in the entry fee. Each player also will be entered into course contests and mulligans are available at $40 per team. Prizes for first, second, third and fourth will be awarded in four flights. There is a maximum of 55 teams and spots will be filled on a first-come, first-serve basis.

The Citizens Memorial Health Care Foundation’s Burgundy Ball is held the third Saturday in September at the University Plaza Convention Center, Springfield. Reinhart Food Service is the presenting sponsor of the evening.
Six hundred people from Bolivar and the surrounding communities attend the event. Speakers have included Lee Ann Womack, Ken Schrader, Roy Firestone, Amy Grant, Henry Winkler, Kyle Petty, Vicki Lawrence, Sugar Ray Leonard, Carl Edwards, Ozzie Smith, Anne Jillian and Kevin Sharp.
Proceeds from the live and silent auctions support programs and services for patients and their families at the Carrie J. Babb Cancer Center at CMH including the future Tremain Family Hospitality House to be located across from the Cancer Center. Tickets are $100 per person. Hospice Lights of Remembrance
Citizens Memorial Hospice holds the Lights of Remembrance “Cheer-city “ Open House and Fundraiser in November. The purpose of Lights of Remembrance is to honor the life of a loved one, support the Hospice philosophy of living life fully, comfortably and dying with dignity and to involve the community in the support of Hospice care. Individuals can make donations of $10 to $500 per light in honor of family and friends, or in memory of loved ones.
The lights decorating the tree are dedicated to those people who are being honored or memorialized. Giving to the Lights of Remembrance program provides donations in our area to ensure that physical, emotional, spiritual and bereavement care is given to all terminally ill patients and their families.
