Free sports physicals offered at two CMH clinics in Bolivar
Two Citizens Memorial Hospital family medicine clinics in Bolivar will offer free sports physicals to student-athletes.
Bolivar Family Care Center will offer sports physicals Monday, July 31, through Friday, Aug. 4, 8 a.m.-4 p.m. No appointment is necessary. For more information, call Bolivar Family Care Center at 417-326-6021. Bolivar Family Care Center is located at 1240 N. Butterfield Road in Bolivar.
Butterfield Park Medical Center will offer free sports physicals Wednesday, Aug. 2, 9 a.m.-3 p.m. Appointments are preferred, but walk-ins are accepted. To schedule an appointment, call Butterfield Park Medical Center at 417-326-7676. Butterfield Park Medical Center is located at 1300 N. Oakland Avenue in Bolivar.
The sports physicals are free on the specified dates and times listed only. On all other days, walk-in or scheduled appointments for sports physicals are $44 (payment is due at the time of service).
For student-athletes who also need a wellness examination, the sports physical paperwork can be completed during the wellness appointment. Most insurance plans cover a wellness exam once per calendar year at no cost. Call your clinic to schedule an appointment and for more information.
