CMH opens CMH Health & Wellness Clinic March 4

Citizens Memorial Hospital launched a new medical clinic March 4. The clinic provides wellness visits for patients without a primary care provider and hospital transitional care services for patients without primary care support.

The CMH Health & Wellness Clinic is located at 2520 S. Springfield Ave., Suite A, in Bolivar. A ribbon-cutting ceremony is scheduled for Thursday, April 4, at 1:30 p.m.

Family nurse practitioner Shelby Bailey is leading the clinic’s medical services. Bailey has extensive experience as a family nurse practitioner in a variety of care settings. She previously worked as a family nurse practitioner hospitalist at Mercy Hospital in Springfield. Most recently, she worked in a critical access hospital as a locum tenen family nurse practitioner hospitalist in Iowa.

According to Michelle Boehne, administrative director of clinics at CMH, the new clinic represents an innovative approach to being more proactive in caring for our patients.

“The hospital transitional care services will provide post-discharge care to CMH’s highest-risk patients who are discharged from inpatient care or the emergency department. We’ll be able to help connect those patients to a primary care provider,” says Boehne. “Additionally, we’ll be able to provide the Medicare annual wellness visit to patients without a PCP and get them started on the road to routine health and wellness care.”

The clinic does not provide walk-in clinic services. For walk-in care, patients are encouraged to go to the CMH Walk-In Clinic at 2230 S. Springfield Ave., Suite H-J, in Bolivar.

CMH operates 34 primary care and specialty physician clinics in Ash Grove, Bolivar, Buffalo, El Dorado Springs, Greenfield, Humansville, Osceola, Pleasant Hope, Stockton and Willard, Missouri.
