Description: The decentralized clinical staff pharmacist is responsible for the safe and efficient drug utilization in the decentralized department through prospective medication order review and verification, clinical pharmacy support and provision of drug information to meet the needs of physicians, nurses, and patients. Duties include assisting with medication reconciliation, patient education, facilitating compliance with core measures, prospective formulary management and appropriate documentation in the medical record. The pharmacist will document all interventions. The ED decentralized pharmacist may provide on-call coverage for the inpatient pharmacy services, assist the director in maintenance of quality and medication management standards, conducting orientation, teaching, and developing in-service training programs. The ED pharmacist will be expected to respond as a member of the code blue team, and will perform other duties as required. Additional pharmacist duties include preparation of compounded sterile products, providing for the security and distribution of controlled substances, assuring the continuous supply of pharmacy stock items to patient areas, and supervising the work of technicians as required. The ideal candidate will have a Doctor of Pharmacy degree, will have completed a residency program in emergency and/or critical care and be ACLS certified. This candidate will have the ability to maintain courteous and diplomatic relations with physicians, nursing staff, and patients, promote good interpersonal and interdepartmental relationships, and behave in a manner consistent with mission, vision and values of CMH.
Shift: EITHER DAY OR EVENING SHIFT
Hours: Full Time position working 80 Hours/Period - This position requires working On Call, Holidays
