Services Provided

The Osceola Rehabilitation Clinic provides outpatient physical therapy services inside Osceola Medical Center and Walk-In Clinic. Physical therapy treats those who have become weak or disabled due to:

  • Illness
  • Injury
  • Pain
  • Loss of coordination, strength, motion or function

Share or Request Medical Records

Records also can be requested by completing and returning the Release of Information Authorization Form PDF via mail or fax. A medical record copying fee will apply. Please allow at least 3-5 business days to fill your request.
