Citizens Memorial Home Care Services has received a five-star rating – the highest rating possible – by the Centers for Medicare and Medicaid Services (CMS) for patient service. The star ratings are based on Home Health Consumer Assessment of Healthcare Providers & Systems (HHCAHPS) survey information and are designed to measure the experiences of people receiving home health care from Medicare-certified home health agencies.

“Our Home Care Services team is truly compassionate and respectful in caring for our patients and I am very proud of them in receiving this five-star rating from the patients they serve,” said Valerie Noblitt, R.N., BSN, Director of CMH Home Care Services.

The home health agency ratings are publicly reported quarterly on Medicare’s Home Health Compare (HHC) website: www.medicare.gov/homehealthcompare/. According to CMS.gov, the website is “as a key tool for consumers to use when choosing a home health care provider. It is designed to be an easy-to-access, convenient source of authoritative information on provider quality.”

In addition to CMH Home Health’s five star rating, CMH Hospice has received 100 percent on five patient service survey questions on Hospice Compare website: www.medicare.gov/hospicecompare/. Two questions were related to patient preferences such as: patients or caregivers who were asked about treatment preferences like hospitalizations and resuscitation, and their beliefs and values at the beginning of hospice care. The remaining three questions were related to managing pain and treating symptoms such as: Patients who were checked for pain at the beginning of hospice care; patients who got timely treatment for shortness of breath; and patients taking opioid pain medication who were offered care for constipation.

CMH Home Care Services is operated by Citizens Memorial Health Care Foundation and provides a variety of medical services in the home including professional nursing, physical therapy, speech therapy, tele-health, infusion therapy, occupational therapy, medical social services, certified nurse assistants, and dietitians. Services are provided to residents in Cedar, Dade, Dallas, Hickory, Greene, Polk, St. Clair and Benton counties.
