Rehabilitation at Lake Stockton Healthcare Facility

In recent years, 40-50% of the residents discharged from LSHCF returned home after a convalescent stay. This is due in large part to an incredible team of therapists that work diligently to help residents recover from a variety of health problems that make life in their own home temporarily unsafe or impractical. The old stereotypes about long term care are vanishing as people realize the new role of nursing homes in the continuum of care.

Our team of Physical, Occupational and Speech Therapists work closely with our nursing staff to implement a plan of care that helps strengthen our residents so they return to their optimal level of function. Whether the plan is to return home or stay with us, we want our residents to enjoy the best quality of life possible. Timely and competent therapy is a key component in achieving those quality of life goals.

All residents of LSHCF receive the therapy they need regardless of payment source. Medicare Part A and Part B pay for a large share of the cost, then private insurance and private funds. Those residents whose care is paid for by Medicaid alone receive the necessary therapy with the facility absorbing those costs.

Another unique feature of our facility is the ability to offer outpatient therapy to non-residents. We often have residents that are able to return home, but still need therapy to regain more function or independence. Those folks are still able to visit our therapy unit and get the services their physician prescribes. We also have people in our community who are recovering from surgery or illness after a hospital stay that take advantage of our talented team of therapists. Payment options for these services include Medicare Part B, private insurance or private pay.

In 2007, 47% of the residents discharged from LSHCF returned home after a convalescent stay. This is due in large part to an incredible team of therapists that work diligently to help residents recover from a variety of health problems that make life in their own home temporarily unsafe or impractical. The old stereotypes about long term care are vanishing as people realize the new role of nursing homes in the continuum of care.
