1982
- The hospital opened its doors on September 28, with the ICU addition completed—the initial first phase of expansion.
1983
- CMH held its first annual health expo to service area communities.
- The swing bed program was added to CMH to provide additional skilled nursing services to discharged patients.
- The Patient Representative Program was established to serve as a liaison between patient and hospital.
1984
- Accreditation received from the Joint Commission on Accreditation of Hospital with no deficiencies.
- CMH added home health services and started home medical equipment programs.

1985
- State health agency approval was given for two advance life support ambulances.
- Mammography services were added to the radiology department.
1986
- The CMH Foundation established as a not-for-profit 501(c)(3) entity, which aids and assists CMH in program and endowment development.
1987
- Hospice program was started to assist the terminally ill.
- Second phase of construction on the hospital was completed with an 8,000-square-foot addition to the ICU/CCU unit, including a new intermediate care unit. Construction also included expansion of the laundry and dining room facilities.
1988
- CMH completed third phase of construction with the addition and dedication of the outpatient wing, which houses outpatient surgery facilities, recovery rooms, clinic rooms and a waiting room and registration area, which allowed specialty physicians to bring needed services to the hospital on a weekly basis. Expansion also included space for community rooms that are accessible to the public, as well as space for rehabilitation services, which includes physical, occupational, speech and cardio-pulmonary rehab. This expansion enlarged the hospital from its original 47,000 square feet to 76,000 square feet.
- Acquisition/merger of the Bolivar Nursing Home by the Foundation and renamed to Citizens Memorial Healthcare Facility.
- First phase of construction of 24 units at Butterfield Residential Care Center was completed.
1989
- CMH was designated as a Level III Trauma Center.
- Stockton Home Medical Equipment and Hermitage Home Medical Equipment satellites opened.
1990
- The Medical Excellence Scholarship Fund was established to give scholarships to students in medical fields.
- CMH acquired Hermitage Family Medical Center in Hermitage and Butterfield Park Medical Center in Bolivar.
- Completed phase II construction of Butterfield Residential Care Center for a 52-unit facility.
1991
- Dedicated the hospital expansion of The Birth Place (women’s health center) and the Parkview Wellness Center.
- In-home infusion therapy was started for Hospice patients.
- Completed phase III of construction of Butterfield Residential Care Center for a 60-unit facility.
1992
- Celebrated tenth year in operation.
- Acquired Dade County Family Medical Center in Greenfield.
1993
- Started the CMH Wellness & Counseling Center in Bolivar.
- Opened state-of-the-art long-term skilled nursing facility, Parkview Healthcare Facility in Bolivar.
- CMH was selected as one of the top ten rural hospitals in the United States.
1994
- Citizens Advantage (HMO) began networking the service area.
- Opened Ash Grove Family Medical Center in Ash Grove and Dallas County Family Medical Center in Buffalo-both rural health clinics.
1995
- Addition of neurology services.
- CHART program, which helps identify healthcare needs in the service area through local coalitions from each community addressing social, spiritual and medical needs, was established and coordinated in Polk, Dallas, Cedar, Hickory and Dade counties.
- Provided free school health programs to area schools—Good Health is More Than Magic. 15,000 elementary students took part.
- According to a survey in the Department of Health’s Obstetrical Services Buyers Guide, CMH’s Birth Place received the highest level of satisfaction from patients.
1996
- CMH became the home of Polk County’s 911 service.
- CMH, along with three other rural healthcare hospitals, merged Citizens Advantage (HMO) into Missouri Advantage (HMO) to expand healthcare coverage and services.
- Mobile MRI unit was added to the services of CMH.
- Home care and rehabilitation services were extended in the Ash Grove area through a contract with the Ash Grove Nursing Home.
- Respiratory care therapy services were added to Humansville and all Foundation long-term skilled nursing facilities.
1997
- CMH participated in the creation of the Barceda Families program to act as the fiscal intermediary. This program is a cooperative effort of over 30 representatives from public and private family resource organizations in Barton, Cedar and Dade counties. This children’s trust fund-sponsored project is a pilot project that demonstrates how the coordination of appropriate community family services can prevent child abuse.
- More than $15,000 was raised at the Medical Excellence Golf Classic for the scholarship fund.
- CMH sponsored a golf tournament in Buffalo, which raised $2,000 for the Dallas County Chapter of Habitat for Humanity.
1998
- CMH participated in the first-annual Relay for Life with the American Cancer Society, which raised over $31,000 for cancer research.
- CMH Foundation began its Health Transit Services program, which offers two 15-passenger para-transit buses with wheelchair accessibility. The service is available to private-pay citizens or Medicaid recipients for medical-related appointments.
- Completed construction and opened the dialysis center in conjunction with the Springfield-based group, the Kidney Disease Center of the Ozarks, Bolivar Satellite.
- Creation of Riverside Management and Rehabilitation, which was established to accommodate Missouri Advantage and expand the rehabilitation department.
- The CMH Sports Medicine Center opened in Bolivar.
- Mental health services were expanded to the rural healthcare clinics.
1999
- Purchased Ash Grove Nursing Home.
- CMH sponsored the Nike Ozarks Open golf tournament with a Saturday pro-am to continue to raise money for the Medical Excellence Scholarship program.
- CMH began the work health program, occupational medicine, which began with CMH employees and then branched out to area businesses.
- CMH facility obtained OSHA’S VPP merit status, becoming the first long-term skilled nursing facility in the nation.
- Women’s health program, Hey Lady, Have a Heart, provided free health screenings and education to over 500 women. The annual event is held every February in conjunction with National Heart Month.
2000
- Completion of a three-year strategic plan for the organization, which brought branding changes to roll Citizens Memorial Hospital, Citizens Memorial Healthcare Foundation, and Riverside Management and Rehabilitation into one: Citizens Memorial Healthcare.
- CMH launched its first Web site.
- Missouri Advantage dissolved in December.
- Miles for Smiles Mobile Dental units received federal funding and began providing dental services to Medicaid children.
- CMH launched employee wellness program.
2001
- CMH’s participation in the Relay for Life (American Cancer Society) earned over $90,000.
- Expansion of the Cardio-Pulmonary Rehabilitation Department added additional equipment, an exercise area and an indoor walking track.
- Ash Grove Family Medical Center opens.
- Citizens Memorial Healthcare Facility, Butterfield Residential Care Center and Parkview Healthcare Facility obtained OSHA’s VPP status of Star, Star and Merit, respectively. CMHF and BRCC were the first facilities in the nation of their kind to receive this top honor. PHF was the third in the nation to do so. Later, Colonial Springs Healthcare Center obtained OSHA’s VPP Star Status.
- Addition of CMH Childcare Center.
2002
- Purchased Community Springs Nursing Home in El Dorado Springs.
- Missouri Sleep Institute, in conjunction with Dr. George Heinz, was initiated bringing a sleep clinic to CMH.
- Ash Grove Senior Living Community began accepting residents in January.
- CMH began a comprehensive upgrade to its technology infrastructure, InfoCare, which allows patients to enter any CMH facility and receive expedited registration based on patient profile data that is already in the system, therefore creating one seamless system.
- CMH celebrates its twentieth year.
