National Long Term Care Week
This week is National Long Term Care Week and with that, we’d like to thank the Foundation staff for their hard work in continually providing high standards of care to our residents, whether they require a short or long term stay. And, thank you to the families who entrust their loved ones into our care. We take great pride in your choice of CMH facilities, and look forward to meeting you at activities hosted throughout the year.
CMH long term care facilities are part of a culture change movement dedicated to providing resident-centered care, rather than creating care plans geared strictly toward a disease or condition. MC5, also known as Missouri Coalition Celebrating Care Continuum Change (www.momc5.com) is an organization whose purpose is to "create senior living environments where each individual’s dignity and value is respected and celebrated." MC5 advocates care plans that allow residents of long term care facilities to make decisions regarding their care at a long term care facility.
At all CMH long term care facilities, our purpose is the same. We look to provide individualized care plans, based upon individual residents. No two residents are alike, and no one’s care plan should be the same as their neighbor. All CMH long term care staff is dedicated to providing activities, educational opportunities, environmental services, and nutritional services that build relationships amongst the residents and staff to create a strong network of security for our residents. That strong network provides choices for residents based upon personal preferences. Upon admission, residents are interviewed by staff to determine when and what they prefer to eat, when they prefer to wake up and go to bed, and how they like to bathe.
